CHESHIRE organisations have formed a collaborative to help transform palliative care.

The Strategic Collaborative Cheshire consists of clinical commissioning groups, including South Cheshire and Vale Royal as well as community trusts, hospitals, hospices, council and clinical networks.

The SCC has launched a plan for palliative and end of life care which is based on a national framework and sets out the collaborative approach needed from health and social care providers.

Sinead Clarke, a GP in the South Cheshire and Vale Royal area, said: “We hope that the plan will inspire and motivate everyone to reach for excellence.

“Working in partnership is crucial to ensure that individuals and those important to them receive the best possible end of life experience and care.”

The views of community members and professionals were sought to help develop the plan and ensure the process was inclusive.
